TBI MOTOR CAR OCCUPANT

Anthony, a 20 year old university student, was out on a Saturday night, partying with friends. Many were drinking. CT scan showed major bruising in multiple areas of his brain, but without focal hematomas. With the respirator, and a device to monitor the pressure inside his brain, he was treated with a number of medications thought to help decrease swelling. He seemed stable enough to have his leg fractures pinned and casted. He remained in a coma for days. He had a number of seizures and was given anti-seizure medicine He regained consciousness very slowly but was totally disoriented and confused. Once the breathing tube was removed and he could talk, he did not make sense. Serial CT scans showed no other brain injury except the many contusions.

Anthony’s family was always at the hospital. Family members read to him and talked to him, also playing his favorite music. Once he was awake, he was evaluated by the rehabilitation team. Rehabilitation was started immediately. Once the doctors felt he was stable enough to leave the hospital, he was transported to an inpatient rehabilitation facility. Even after two weeks there, Anthony was as best at a Rancho Los Amigos Cognitive Scale 3 or 4. Rehabilitation continued in the facility. Eventually Anthony was able to return home, but required full-time care and assistance. His long-term prognosis is poor. Although he may eventually be able to be left alone at times and handle simple tasks, he will not return to the university. He may never be able to live alone or work in any significant capacity.
